Traumatic Diaphragmatic Hernia

Abstract
A case of traumatic diaphragmatic hernia diagnosed and treated 19 years after injury is reported. The patient, 37-year old male, received a contusion on the left lower chest and a fracture of right femur by traffic accident 19 years ago. Two years later, he began to have cough with sputum, dyspnea and occasional epigastric disconfort, being increased in severity ever since. He underwent thoracotomy through 7th intercostal space on november 15 th, 1965 and . a diaphragmatic defect of about 15 cm in length was closed with two layers of interrupted silk sutures. Postoperative course.was uneventful. The train of events after crush or penetrating injury of the abdomen or chest involving the diaphragm is unpredictable. Hours or months or even years might have. elapsed before signs of a diaphragmatic hernia appeared. Chest raentgenograms are the only sure way of making an early diagnosis and prompt surgical repair gives the patient his best chance of recovery.
